ByHeart says all lots of its powdered infant formula may be contaminated after an independent lab found Clostridium botulinum type A in 5 of 36 tested samples, prompting the company to expand a voluntary nationwide recall, urge parents to stop using the product and offer refunds for online purchases. The FDA says at least 31 infants in 15 states have been hospitalized in the outbreak (no deaths reported); investigations and wider testing are ongoing and recalled product has been found on store shelves despite the recall.
